Illinois will change both name and sex, and will issue a new birth certificate rather than amend the old one. Previous information stated that the new birth certificate contained the words “copy of the original,” but the certificate does not say “copy” now.

There’s a new revision of the form, out in 2005, which requires either a letter from a surgeon licensed to practice in the United States, or an examination, post op, by a physician licensed to practice in the United States.

Request them to fax you what you need to get signed by the doctor and notarized, top and bottom (see below). Make sure you specify name and sex aka gender change form. They can also mail it to you snail mail; but it is easier to get it faxed, signed, notarized, and send in $15.00 for one copy, $2.00 for each additional copy.

A third alternative is to call (217) 782-6554, asked for Vital Records, then ask for the birth certificate change department. This is not a recommended method though, as some individuals will resist offering assistance. If you do persist, ask for a “request for change of name and sex form.”

“That the following were the personal particulars at the time of birth of _________________” Complete this line using (i.e. John Doe, now Jane Doe) or the opposite, if appropriate. Complete this form and return it to the below address:

IMPORTANT: Send it by Priority mail

Include the original letter from your SRS surgeon (unless the surgeon completes the top part of the form as above) and a certified copy of the name change order, and a check for $15.00 made out to “Vital Records”. Additional copies are $2.00 each.
