CONNECTICUT Statute: Conn. Gen. Stat. 19a-42 (2003)
Connecticut will change both name and sex.

A court order is not required. Documentation from your SRS surgeon is required and the fee is $15.00.

(a) To protect the integrity and accuracy of vital records, a certificate registered under chapter 93 may be amended only in accordance with sections 19a-41 to 19a-45, inclusive, chapter 93, regulations adopted by the Commissioner of Public Health pursuant to chapter 54 and uniform procedures prescribed by the commissioner. Only the commissioner may amend birth certificates to reflect changes concerning parentage or any change. Amendments related to parentage or gender change shall cause the creation of a replacement certificate that supersedes the original and shall in no way reveal the original language changed by the amendment. Any amendment to a vital record made by the registrar of vital statistics of the town in which the vital event occurred or by the commissioner signed in accordance with such regulations and uniform procedures.
