Oregon will change both name and sex, and will issue an amended birth certificate, without any designation of what has been amended.

The requirement for the gender change on birth certificates requires a court ordered gender change, which can be done by crossing out the word “name” and inserting “gender” on the court “name change” form

CODE:
(3) Upon receipt of a certified copy of an order of a court of competent jurisdiction indicating that the sex of an individual born in this state has been changed by surgical procedure and whether such individual’s name has been changed, the certificate of birth of such individual shall be amended as prescribed by rule of the state registrar.
