New Mexico
NEW MEXICO Statute: NM State Ann 24-14-25 (2004)

New Mexico will change both name and sex, and will issue a new birth certificate rather than amend the old one. The old information will be sealed and cannot be opened without a court order.

You will need an original letter from your SRS surgeon and an original or certified copy of the court order for your name change. The fees are $10.00 for changing the information and another $10.00 for a new certificate.

CODE:
Upon receipt of a duly notarized statement from the person in charge of an institution or from the attending physician indicating the sex of an individual born in this state has been changed by surgical procedure, together with a certified copy of an order changing the name of the person, the certificate of birth of the individual shall be amended as prescribed by regulation.
