The following is the most current information, as of March 2002.

Contact the Vital Statistics, New Brunswick office on-line to change name. They have all the information and you can order the “kit” on-line. Provide a letter from a therapist to verify the nature of your request. Also, provide an original ‘long-form’ birth certificate, if born outside the province.

New Brunswick: http://www.gnb.ca/0379/change-e.asp (to change name). It will be an additional $100.00 payable to PNB for name change order

Nova Scotia: http://www.gov.ns.ca/snsmr/paal/vstat/paal306.asp. In order to change name and sex, you will need to supply a change of name certificate, a letter from your SRS surgeon and a letter from another physician who has examined you post op. I recommend all your letters, etc. be notarized.
